On or before your first visit, you will be required to complete patient information forms. This information is necessary to assist us in filing your insurance claim or
handling your particular situation. A consent to release information and authorization for your insurance company to pay our facility for services rendered to
you are also included. All information you provide us is strickly confidential.
If you would like to schedule a new patient appointment, please call or text our office at (337)478-9331 and a link will be generated to access these forms.
Completion of all applicable forms is necessary for your first scheduled appointment with your provider, Failure to complete your new patient forms before your scheduled appointment
time will result in your appointment being rescheduled.
Patients are seen by appointment only. Appointment schedules vary according to provider availability. We are happy to reserve a time in your provider's schedule, however, in consideration for others,
we do request at least a 24-hour notice prior to cancellation of any appointment. We do understand that there are circumstances that may prevent you from keeping
your appointment. If you provide us with as much notice as possible, we may be able to contact another patient who needs an appointment. Appointments fill quickly,
and cancelling with less than 24-hours notice does not allow enough time to schedule another patient in need of treatment. Therefore, a cancellation or no-show fee
will apply if our office is not notified that you will be unable to make your appointment.
At the time of your appointment, you are required to present a copy of your drivers license and all insurance cards. Failure to do so will make you responsible for paying for your visit in full.
Any patient financial responsibilities are due at the time of service. For your convenience we accept cash, checks and credit cards.
